Prescription Orders
KV Vet Pharmacy

Raymond Metzner, DVMThe KV Vet Pharmacy makes buying prescription medications for your animals easier and more affordable. To order online, simply complete the "Prescription Wizard" found on the product page. After answering a few simple questions, you may add the item to your basket and continue shopping as usual.

KV requires authorization from your veterinarian to process any prescription order. If you place your order before contacting your vet, please give your vet the order number and ask them to include this when calling or faxing. This allows us to easily identify your order. All prescription orders are held pending authorization from your veterinarian.

Aaron Stutzman, Pharm. D, RPHow do I set up a Prescription File with KV Vet Supply?

By Fax from the Veterinarian: (most preferred)

Have your veterinarian fax a copy of your prescription from their office to:

800-269-0093

* For orders already on hold, please have your veterinarian include your Order Number with their fax.

Over the Phone:

1. Have your Veterinarian call in your prescription at 800-423-8211: prompt #3.

2. A KV pharmacist or pharmacy technician will set up your prescription file.

* For orders already on hold, please have your veterinarian reference your Order Number.

By Mail:

1. Obtain the written prescription from your Veterinarian.

2. Send the original prescription to:

KV Vet Supply
Attn: Pharmacy
PO Box 245
David City, NE 68632

* For orders already on hold, please include your Order Number.

If you have a veterinarian's license or a pharmacy license:

Please fax a copy of your current state license to 800-269-0093. Once approved, your license enables you to purchase any prescription medication we offer until that license expires.

* For orders already on hold, please include your Order Number.

NOTE: Prescriptions remain on your file for one year from the date that script was written by the veterinarian. As long as you still have refills on your prescription, you may order them at any time within one year. After that time, your veterinarian will need to authorize a new prescription.
